Overview The Admission RN assesses patients in their place of residency or in hospitals and initiates care into the services of Chapters Health System, ensuring a seamless transition to Care Navigation and serving as an ambassador of the organization. Benefits $7,000 sign‑on bonus Generous PTO Mileage reimbursement Comprehensive orientation Tuition reimbursement Flexible shift options (5–8-hour or 4–10-hour shifts) Supportive team and leadership Qualifications Current RN license in the state of employment Minimum one year nursing experience; hospice or hospital experience preferred Previous experience with an EMR/EHR system Strong clinical assessment skills; preferred background in emergency department or ICU Effective communication in English (verbal and written); bilingual in Spanish preferred Professional, articulate presentation of services Valid driver’s license and automobile insurance per company policy Reliable transportation Teamwork and appropriate intervention skills Ability to manage multiple priorities and handle emotional stress Capability to perform essential physical functions (lifting patients/equipment, bending, pushing/pulling, kneeling) with or without reasonable accommodation Goal/outcome orientation with strong time‑management skills Excellent customer‑service focus Active BLS certification from American Heart Association or Red Cross Responsibilities Complete clinical admissions for all company services Explain services to patients/families and address questions and concerns, presenting in an empathetic, compassionate manner Identify admission objections or barriers and provide responses Educate patients/families on admission requirements and address immediate needs Complete informed consents and initial assessment per regulations and company policy; provide information to physicians and initiate Plan of Care to address patient’s immediate needs Initiate skilled nursing interventions to prevent complications, alleviate symptoms, and maximize comfort Obtain physician orders Complete documentation per company policy Act as company representative at assigned facilities, facilitating referrals to all service lines and collaborating with hospitals, physicians, facilities, patients, families, and the public Communicate with Care Navigation staff to ensure timely follow‑up; notify the team when assignment is complete and prepare for the next assignment Provide timely clinical communication using SBAR Discuss potential needs with after‑hours staff Obtain consents for services and arrange safe discharge efficiently for referred and eligible patients while meeting productivity expectations Obtain medical record information for the referral Develop strong relationships with case managers, physicians, and other staff at facilities Work with Professional Relations Representatives to provide education on all available services Provide care for any GIP patients in a hospital, including daily visits, professional management, documentation, and efficient discharge In emergencies (e.g., hurricanes), report to work at a company‑designated location, possibly before scheduled time, and stay overnight as needed Perform other duties as assigned Physical Demands While performing duties, the role requires abilities to see, hear, talk, walk, use hands.
