We are seeking a dedicated and compassionate Social Worker to join our team at Ocean Grove Post Acute in the beautiful town of Millsboro, DE.

Salary Range: $55,000-65,000 per year based on experience

As a Social Worker at Ocean Grove Post Acute, you will play a vital role in providing exceptional social services to our residents. You will be responsible for planning and implementing social services programs that cater to the unique needs of our residents. Responsibilities:

• Demonstrate knowledge of age-specific developmental factors specific to adult residents in planning social services programs.
• Plan and implement social services programs that address the needs of residents with various conditions, including therapeutic, neurological, and dementia.
• Evaluate and assess the needs of prospective residents to determine the appropriateness of facility placement.
• Promptly follow-up on admission inquiries and ensure a smooth transition for new residents.

Requirements:

• Experience in social work or a related field
• Bachelor's degree in Social Work or a related field preferred
