This requires you to be able to learn asynchronously, as well as raise your hand when help is needed, or gaps are identified The Family Advocate (FA) is a vital member of the Nest Health interdisciplinary care team with training and experience as a Medical Assistant. The FA provides direct patient care to adults and children in community and home settings under the supervision of a licensed clinician and plays a central role in connecting Nest families to the care, community resources, and support they need. This is a hybrid role combining in-person clinical support, virtual patient engagement, and supply and logistics coordination. It is a great fit for someone who is organized, compassionate, and energized by working directly with families in their communities. Adhere to infection control standards including hand hygiene, PPE use, and isolation protocols Lead intake for virtual new and existing family visits; introduce the program, collect full medical and social history and consents, and complete screeners before the provider joins Complete accurate and timely EMR documentation per Nest Health requirements Help families identify and access health and social care resources, including food, housing, transportation, and financial assistance, and follow up to confirm they were able to connect coordinate with management for replenishment as needed Transport and set up exam and procedure equipment at community locations; On virtual days, support provider panel management by contacting families to schedule annual wellness and well child visits, and following up with families to confirm access to social care resources and specialist referrals Assist with patient registration, prior authorization approvals, and administrative workflows, including medical records, diagnostic report, and referral information management Actively participate in compliance activities including equipment checks and lab, medication, and vaccine inventory Participate in community outreach to support family enrollment and program awareness Completion of an accredited Medical Assistant program High school diploma or equivalent; At least 1 year of experience with vital signs and phlebotomy (preferred)[RA2] [JR3] Fluency in English required; bilingual candidates strongly encouraged to apply Experience in community health, community outreach, or social services in a community-based organization (preferred)

• What are the travel requirements?

This is a hybrid role providing in-person care in community and home settings, as well as virtual patient engagement from home. You will travel to select sites in the service area in both the greater Flagstaff & Yavapai regions as the program grows. Physical requirements include prolonged periods at a computer during virtual work, transporting and lifting clinical supplies and equipment up to 25 lbs., and setting up and breaking down portable exam equipment at clinic locations.
