Case Coordinator

The coordinator's primary responsibility is to work directly with our caregivers to ensure they are providing our clients with the compassionate care our clients deserve. Coordinators focus on delivery of care through scheduling and continuous communication with client's and caregivers. Qualifications Working knowledge of HHA Exchange Excellent organizational and time-management skills At least 3-5 years of experience in a Home Care setting Great communication and interpersonal skills Attention to detail with problem solving skills Strong customer service background Staffing open cases and providing daily support to clients and caregivers Requirements Staff personnel according to home care patient's schedules Documents and reports to Case Manager when there is a change in client status, change in service requirement, change in aide assigned to case, client, nurse incidents, and complaints.
