What You’ll Do

As a Program Case Manager, you will provide compassionate support and comprehensive case management to individuals participating in Interfaith programs. You will help clients identify their goals, connect with resources, overcome barriers, and take meaningful steps toward stability.

You will:

• Welcome and support clients through intake and participation in Interfaith programs.
• Build trusting, respectful relationships with individuals from a variety of backgrounds and life circumstances.
• Provide encouragement, guidance, and practical support as clients work toward their goals.
• Help clients identify strengths, develop coping strategies, and overcome barriers related to addiction, employment, grief, and other life challenges.
• Help clients explore, obtain, and maintain employment.
• Educate and connect clients with community resources that can support their long-term success.
• Coordinate with nonprofit organizations, governmental agencies, treatment providers, schools, and other community partners to create a strong network of support around each client.
• Participate in and/or lead programs such as Pathways, grief support groups, SMART Recovery , and other groups as programs develop.
• Follow up with clients and community partners to help ensure that individuals receive the support they need.
• Advocate for clients while encouraging independence, personal responsibility, and positive growth.
• Help identify new opportunities and creative solutions that strengthen Interfaith's programs and the people we serve.
• Perform other duties that support Interfaith's mission and are assigned by the Executive Director.

Minimum Qualifications

• Undergraduate or graduate degree; preferably in social work, human services, counseling, psychology, nonprofit management, or a related field.
• Education or experience in social services, case management, nonprofit work, or a related field.
• Excellent communication and customer service skills.
• Strong listening, relationship-building, and interpersonal skills.
• Ability to work independently while also being a collaborative member of a team.
• Strong problem-solving and critical-thinking skills.
• Ability to maintain appropriate professional boundaries and confidentiality.
• A genuine desire to help people move toward greater stability and success.
