Position Overview

We are seeking a dedicated Licensed Independent Clinical Social Worker (LICSW) to join our team specializing in hospice care, based remotely in Bellevue, Washington. This full-time position allows you to work Monday to Friday, from 9:00 AM to 5:00 PM, providing essential clinical social work services to patients and their families. In this role, you will be pivotal in ensuring continuity of care, guiding families through complex emotional landscapes, and facilitating effective discharge planning.

Work Environment

This position is fully remote, allowing for flexibility while maintaining a direct connection with our team and clients. Our Hospice Center operates with a patient-centered philosophy, and our social workers utilize digital tools to connect with patients and families, coordinate with healthcare providers, and manage case documentation effectively. You will have the opportunity to engage in regular video conferences and team meetings to foster collaboration and ensure optimal patient outcomes.

Key Responsibilities

Your primary duties will include:

  • Managing a caseload of patients, employing evidence-based techniques to address the social, emotional, and psychological needs of patients and families.
  • Conducting comprehensive assessments to identify key factors affecting patients’ wellbeing, including environmental, financial, and cultural influences.
  • Developing, implementing, and reviewing individualized care plans in collaboration with a multidisciplinary team to ensure the best outcomes for patients.
  • Providing counseling and support to patients and their families, helping them navigate the challenges associated with serious illness and end-of-life care.
  • Coordinating discharge planning and post-discharge follow-up to ensure continuity of care.
  • Documenting patient interactions and care plans meticulously to maintain a legal record and contribute to the organization’s quality assurance efforts.
  • Participating in community outreach initiatives to promote advocacy and support for hospice services.
  • Staying abreast of best practices in social work and hospice care to continually improve the quality of services provided.

Patient Population / Client Focus

In this role, you will primarily interact with a diverse patient population facing terminal illnesses. You will support individuals of various ages and backgrounds, including those dealing with complex emotional and behavioral challenges. You must demonstrate a genuine understanding of the unique needs of different demographic groups, such as children, the elderly, and those with severe psychological distress, tailoring your approach to meet their specific needs.

Qualifications

To succeed in this position, candidates must meet the following requirements:

  • A Master’s degree in Social Work from an accredited institution.
  • A current and valid Licensed Independent Clinical Social Worker (LICSW) license in Washington State.
  • Minimum of 2 years of professional experience providing social work services, preferably in a hospice or hospital setting.
  • Proficiency in working with electronic health records and case management software.
  • Demonstrated ability to manage stress and maintain professionalism in challenging situations.
  • Outstanding verbal and written communication skills necessary for effective interaction with patients, families, and interdisciplinary team members.

Preferred Experience

While not required, the following qualifications are considered advantageous:

  • Experience with bereavement support and end-of-life issues.
  • Knowledge of local community resources that can assist patients and families during the hospice journey.
  • Familiarity with trauma-informed care principles and evidence-based therapeutic techniques.

Compensation and Benefits

Black Mountain Mental and Counseling Services offers a competitive annual salary ranging from USD 115,000 to 130,000, commensurate with experience. In addition to salary, we provide an attractive benefits package including:
