Job Title: Registered Nurse (RN) Case Manager – Home Health | Your Patients Deserve More Than a Rushed Visit Territory: Burnsville, Eagan + 35 mile radius from home Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses The Best Nursing Happens When You Actually Have Time to Nurse. What You'll Do Manage a home health caseload throughout Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall. Complete comprehensive OASIS assessments and individualized care plans. Deliver skilled nursing care and patient education. Collaborate closely with physicians and interdisciplinary team members. Help patients recover, remain independent, and avoid unnecessary hospitalizations. Maintain accurate clinical documentation. What We're Looking For Active Minnesota Registered Nurse (RN) license. 3+ years of RN experience (home health experience preferred). Strong assessment and critical thinking skills.
