• Schedule: 8-hour shifts, Monday through Friday, 8:00 AM - 4:30 PM
• On-Call Requirement: Weekday on-call 4:30 PM - 8:00 AM; weekend on-call Saturday 12:00 AM - Monday 8:00 AM
• Productivity Expectation: Approximately 25 visits per week
• Coverage Area: Within 50 miles of Springfield, MO

Job Requirements

• Active Registered Nurse (RN) license
• BLS certification required
• Associate degree in nursing required
• At least 1 year of nursing experience required; hospice, homecare, or case management experience preferred
• Experience with hospice care or end-of-life support strongly preferred
• HCHB experience is a plus
• Strong communication, organization, and time-management skills
• Ability to work independently while collaborating with an interdisciplinary team

Responsibilities

• Serve as an RN Case Manager providing quality hospice care to patients in the home setting
• Assess patient needs, develop nursing plans, and coordinate ongoing care
• Collaborate with the interdisciplinary group to support patient goals and family needs
• Participate in IDG meetings every other Wednesday
• Complete scheduled patient visits and maintain documentation accurately and timely
• Support patients and families with compassionate education, communication, and symptom management
• Provide reliable on-call coverage as scheduled
• Work effectively across the assigned service area while maintaining high-quality patient care
