Acute Hospital Social Worker

This is a 13-week contract position.

Day shift, 5x8 hours (08:00 - 16:30). Weekend requirement: Every other weekend.

We are seeking an experienced Acute Hospital Social Worker to assess patients and coordinate care. The ideal candidate will possess knowledge regarding drug/alcohol abuse treatment options, experience with patients experiencing homelessness, as well as those with mental health history or in crisis. This role involves organizing and participating in family meetings and contributes to the development of treatment plans.

Responsibilities:

• Conduct assessments and care coordination consultations.
• Participate in the development and revision of treatment plans.
• Educate patients and caregivers to facilitate discharge.
• Document patient interactions according to care delivery plans.
• Engage with multi-disciplinary teams to ensure continuity of care.
• Handle cases involving patients with mental health issues or crisis situations.

Required Experience / Certifications / Licensure:

• Minimum of 2 years of acute hospital social work experience.
• Pending California state license accepted.
• Experience with assessments, case management, and collaborative care.
