Licensed Clinical Social Worker (LCSW)/Licensed Professional Counselor (LPC)/Licensed Master Social Worker (LMSW)The Licensed Clinical Social Worker (LCSW)/Licensed Professional Counselor (LPC)/Licensed Master Social Worker (LMSW) is a part of the patient care team whose primary purpose is to work in conjunction with the health services within the health center in order to provide assessment, diagnosis and treatment planning of behavioral health to children, adolescents, adults, and families in order to improve psychosocial functioning.Responsibilities:Assists in the development of a system to assess and intervene with high-risk patientsDevelops overall resource requirements to meet identified needsDevelops and implements policies, procedures and guidelinesUtilizes current research findings to support clinical social work services best practices on an ongoing basisParticipates in the delivery of care as scheduledPerforms and documents patient's screenings, psychosocial assessments, progress and referral informationAssists in the process of advocacy for patient's access to resources through community programs and coaching to the patients, when appropriateCommunicates treatment plan to patients, families and appropriate staffCollaborates and coordinates with the caregiver, medical provider, service assessment, case management, home health care and all other disciplines necessary to provide the best care to patientsProvides group and individual counseling to patients on related psychosocial issuesDemonstrates a commitment to service, organization values and professionalism through appropriate conduct and demeanor at all timesMaintains confidentiality and protects sensitive data at all timesAdheres to organizational and department specific safety standards and guidelinesWorks collaboratively and support efforts of team membersWill not allow any organizational practice, activity, decision or circumstance which is unlawful, negligent, contrary to the mission, vision or policies of the Mid-Delta Health SystemsQualifications:Must have the ability to work effectively independently and in a team environmentExcellent interpersonal skillsExcellent verbal and written communication skillsRequires professionalism and ability to deal effectively with patients, families, caregivers and a variety of internal and external resourcesMaster's Degree (MSW) in social work from an accredited institution requiredClinical Social Worker licensed by the State of ArkansasMinimum of three years of experiences in a healthcare setting requiredBilingual in English/Spanish preferredSalary Position: The salary for this position is negotiated between the individual and the Chief Executive Officer within the guidelines of the approved MDHS salary plan.Status: This is a FLSA Exempt position. This is an OSHA low risk position. This is an ADA negotiable position. This is a safety sensitive position. Mid-Delta follows state and federal vaccine recommendations.
