Overview

Responsibilities
- Administer prescribed medications and document administration accurately per policy and procedure
- Monitor vital signs, assess resident conditions, and report changes to the supervising clinician
- Provide routine nursing care and assist residents with activities of daily living
- Assist with wound care, catheter care, and treatment administration as directed
- Collaborate with the interdisciplinary team to implement and adjust care plans
- Maintain accurate documentation, ensure patient safety, and adhere to infection control standards

Requirements
- Active LPN license (state of Vermont) or eligibility for licensure in Vermont
- Graduation from an approved LPN program (LPN diploma)
- Basic Life Support (BLS) certification or willingness to obtain
- Prior experience in long-term care or a skilled nursing setting preferred
- Ability to work 40 hours per week; availability for day or evening shifts
- Clear communication and teamwork skills; ability to follow facility policies and regulatory guidelines
