Requirements

• Bachelor's Degree in Nursing (BSN) - Preferred
• Registered Nurse (RN) licensure in the state of practice - Required
• Case management certification - Preferred
• Basic Life Support (BLS) certification from approved vendor per policy - Required
• At least two years recent experience in the field - Required

Responsibilities

• Provide excellent patient care by assisting in the development and implementation of the case management program.
• Act as a liaison between the patient, family, healthcare team, and patient's benefactor to optimize outcomes.
• Coordinate a continuum of care for a defined population of patients from pre-admission through post-discharge.
• Ensure patients and families proceed efficiently through the course of hospitalization.
• Monitor the appropriateness and timeliness of patient care across various disciplines.
• Collaborate with the healthcare team to ensure that care plans are consistent with clinical needs and covered services.
