Pediatric Home Health Registered Nurse

Love to Life Foundation, Inc. (licensed Illinois nonprofit home nursing agency) seeks compassionate, skilled, and dependable Registered Nurses (RNs) to provide in-home care to medically complex pediatric clients in alignment with IDPH, DSCC, Medicaid, and Medicare standards. As a Pediatric Home Health RN, you will deliver holistic, skilled nursing care directly in the client's home, support medically fragile children with ventilators, G-tubes, trachs, seizure protocols, and individualized treatment plans, and collaborate with caregivers and interdisciplinary teams. This is a mission-aligned role for nurses called to nurture life, restore dignity, and deliver world-class care to families who rely on skilled home nursing to safely thrive outside of the hospital setting.

Responsibilities

• Provide skilled nursing interventions in accordance with the physician's orders and individualized care plan (medication administration, wound care, respiratory care, seizure support, etc.)
• Complete nursing assessments at the beginning of each shift and document observations clearly in client records
• Follow infection control protocols and report any signs of illness, abuse, neglect, or changes in condition to the agency and physician
• Perform catheterization, suctioning, enteral feeding, and medication administration via G-tube/J-tube with excellence and safety
• Submit timely electronic clinical documentation using the agency's secure system (Notion or equivalent HIPAA-compliant platform)
• Coordinate with agency RN supervisors for scheduled supervisory visits and plan-of-care reviews
• Adhere to IDPH, Medicaid/Medicare, and DSCC care documentation, licensure, and training compliance requirements
• Educate and support families and caregivers in-home to ensure continuity and confidence in care
• Participate in emergency preparedness plans and trip protocols as needed
• Maintain up-to-date CPR/BLS certification and complete required CEU and annual training hours as mandated by IDPH (77 Ill. Adm. Code 245.70)
