Position Overview We are seeking a relationship-driven, clinically strong Clinical Liaison to serve as the bridge between referral sources, patients, families, and our rehabilitation hospital. In this role, you will evaluate referrals, guide the admissions process, educate partners on the value of inpatient rehabilitation, and help grow awareness of our services across the community. This is an ideal opportunity for a licensed clinician who combines patient advocacy with business development skills and thrives in a fast-paced, field-based environment. What You'll Do •Evaluate referred patients and determine whether they meet admission criteria for inpatient rehabilitation. •Build and maintain strong relationships with physicians, case managers, discharge planners, and other referral sources in your assigned territory. •Educate patients, families, and referral partners on rehabilitation services, clinical programs, and admission pathways. •Coordinate timely, accurate communication throughout the referral-to-admission process. •Develop and execute strategic outreach plans to grow referrals and expand market presence. •Conduct regular marketing calls, presentations, tours, and follow-up activities with community and healthcare partners. •Collaborate closely with internal clinical, admissions, and leadership teams to support patient-centered transitions of care. What You Bring: One of the following active clinical licenses is required: RN, LPN/LVN, PT, OT, or RT.
