Acute Care Physical Therapist Hospital Rehab
Advocate Health Care
Advocate Health Care is seeking a Physical Therapist for acute care staffing on a per diem/PRN basis. The team includes PT, OT, SLP, and rehab aides and serves adult patients across the hospital, excluding inpatient rehab floor and pediatrics.
The role requires evaluating referrals, designing and adjusting treatment plans, documenting patient progress, and coordinating discharge planning and home programs with patients and families.
