Work Environment\nThis role will be performed in a remote setting, providing flexibility and the ability to connect with clients across Hillsboro, Oregon and beyond. As a part of our hospice center's clinical team, you will work closely with other healthcare professionals, caregivers, and community resources to ensure that clients receive comprehensive support tailored to their unique needs. This collaborative approach enhances your ability to create effective treatment plans and improve patient outcomes.
Key Responsibilities\n- \n
- Provide direct clinical support in both virtual and community settings to clients experiencing major mental illnesses and severe emotional disorders. \n
- Active participation in delivering coordinated care within a multidisciplinary team, demonstrating collaboration and achieving shared treatment goals. \n
- Conduct thorough assessments using evidence-based approaches to identify client needs and develop integrated care plans. \n
- Implement crisis intervention strategies and facilitate immediate support as needed, ensuring safe environments for all clients. \n
- Maintain accurate and timely documentation of client interactions and treatment plans, adhering to the organization’s ethical guidelines and standards. \n
- Advocate and connect clients with community resources, ensuring accessibility to a full range of services and support systems. \n
- Carry an established caseload, providing individual, group, and family therapy focused on presenting issues and client goals. \n
- Complete all required reports and data submissions efficiently and accurately, supporting the overall mission of continuous quality improvement. \n
- Provide on-call coverage during assigned hours, ensuring that crisis support is available when clients need it most. \n
- Engage in ongoing professional development and training opportunities to enhance skills and knowledge related to client populations. \n
The successful candidate will work primarily with clients facing significant mental health challenges, often stemming from life-altering conditions associated with end-of-life care. You will focus on delivering compassionate support, addressing the psychological and emotional needs of patients and their families, while fostering a holistic view of health that encompasses physical, emotional, and spiritual well-being.
Qualifications\n- \n
- A Master’s degree in Social Work (MSW) or a related field from an accredited institution is required. \n
- A current Licensed Clinical Social Worker (LCSW) license in the state of Oregon. \n
- Exceptional communication skills, both verbal and written, with the ability to engage effectively with diverse populations. \n
- Robust assessment and crisis management skills, enabling you to navigate complex client scenarios. \n
- Proficiency in organizational and planning abilities to manage caseloads efficiently. \n
- A commitment to ethical practice and adherence to professional codes of conduct. \n

- At least 2 years of clinical experience is preferred, particularly in mental health or hospice-specific contexts. \n
- Bilingual proficiency in English and Spanish is preferred, enhancing communication with a broader patient base. \n
