Non-Medical Case Manager FT / PT Status – Full-Time Salary – $21.48 - $28.00 Hourly Wage Remote Work / In-Office – Hybrid Application Deadline – 8/10/2026 About the Role Location – 288 North 1460 West Salt Lake City, Utah, 84116 United States Background Check Required – Yes Drug Test Required – No Schedule Code – TL - Time limited Key Responsibilities Toprovide HIV non-medical case management to make sure Ryan White Part B clientsenter and remain in HIV medical care and have access to and are adherent ontheir HIV medications. Provide case management in accordance with the Case Management Service Standards for the Utah Ryan White Part B Program Perform client intake Assist in application and re-certification for the Part B Program Assess needs utilizing assessments as provided by the Program Develop care plan Coordinate care and benefits Provide and track referrals Provide internal and external resources Perform follow-up as needed and indicated by acuity and assessment scores. Qualifications Minimum or day-one qualifications: Oneyear of paid employment/professional experience providing case managementservices, excluding internships, AND one of the following: Bachelor of Social Work. Bachelor degree in health orhuman services. OR Five years of paidemployment/professional experience providing case management services,excluding internships. Workingknowledge of web-based applications and Google Workspace, MicrosoftOffice, or similar. Ability to work independently with minimalsupervision Preferred or trainable/on-the-job qualifications: Previous case management experience Working knowledge of HIV Knowledge of or experience with Ryan White Part B Familiarity with social service and support systems within Utah Experience with ClientTrack data management solution Supplemental Information Effective July 1, 2021, State of Utah Employees must either reside in Utah or be in the process of relocating to within 30 days of their start date.
