Care Manager

Work Arrangement: Field-Based / Member-Facing Employment Type: Contract Duration: 6-Month Contract with Potential for Extension or Conversion Pay Rate: $40.00/hr – $45.00/hr on W2 Application Deadline: 6/3/2026 Skills Required Primary (Must-Have) Degree from an Accredited School of Nursing or Bachelor’s Degree in Nursing 2–4 years of nursing, case management, or home health experience Strong care management and patient assessment skills Ability to develop personalized care plans and provide member education Strong communication and interpersonal skills Ability to work independently and manage changing priorities effectively Strong critical thinking and problem-solving abilities Ability to complete documentation accurately within required timelines Comfortable conducting member home visits and field-based work Ability to travel within assigned service delivery areas Secondary (Good to Have) Experience in member-facing or community-based healthcare roles Experience coordinating healthcare services and resources Strong teamwork and collaboration skills Flexible and adaptable approach to changing healthcare environments Experience working in managed care or population health settings Position Overview The Care Manager RN will develop, assess, and facilitate complex care management activities focused on supporting members with primarily physical healthcare needs. This field-based role involves conducting home visits, coordinating care plans, providing education to members and families, and ensuring high-quality, cost-effective healthcare outcomes. The ideal candidate will possess strong clinical judgment, communication skills, and the ability to work independently while collaborating with interdisciplinary healthcare teams. Develop and implement personalized care plans for members based on healthcare assessments and needs Conduct member home visits and provide ongoing care management support Facilitate complex care management activities to support quality healthcare outcomes Educate members and families regarding healthcare plans, treatments, and available resources Review emails, tasks, voicemails, and respond to urgent member needs daily Coordinate and manage 5–7 member visits weekly within assigned service delivery areas Complete required documentation accurately within 24 hours of member interactions and visits Collaborate with healthcare providers, care teams, and community resources to support member care needs Monitor member progress and adjust care plans as needed to improve health outcomes Meet required turnaround times for care management processes and assigned training completion Participate in virtual training sessions with camera participation required during onboarding Support organizational goals related to member independence, care quality, and service excellence Work collaboratively with leadership and coworkers to provide exceptional member support and care Benefits Medical Dental Vision 401(k) Paid Sick Leave EEOC Compliance We are an equal opportunity employer, and all qualified applicants will receive consideration for employment. #J-18808-Ljbffr
