Job Summary

We are seeking an experienced Licensed Practical Nurse (LPN) with Long-Term Care and Skilled Nursing experience for a 13-week travel assignment. The ideal candidate will provide compassionate, high-quality patient care while collaborating with physicians and the interdisciplinary healthcare team.

Key Responsibilities

• Administer medications and treatments as prescribed.
• Perform patient assessments and monitor resident conditions.
• Assist with care planning and implementation.
• Maintain accurate and timely documentation in the EMR system.
• Coordinate care with physicians and other healthcare professionals.
• Provide resident-centered care in a fast-paced long-term care environment.
• Work assigned schedules, including weekends and holidays as needed.

Requirements

• Active Hawaii LPN License
• Current BLS/BCLS Certification
• Minimum 1 year of recent Long-Term Care or Skilled Nursing experience
• Experience using electronic medical records (EMR)
• Strong communication, organizational, and clinical assessment skills
• Ability to work independently and as part of a multidisciplinary team
