Clinical Pharmacist - CCM, TCM & RPM Lead

Axil Health is seeking a dedicated Clinical Pharmacist to lead Chronic Care Management, Transition Care Management, and Remote Patient Monitoring services across North Carolina. You will build and work directly with patients, managing their care and driving clinical outcomes. The ideal candidate will have a PharmD Degree, a valid pharmacist license in North Carolina, and experience in patient management.
