This position serves as a critical resource within multidisciplinary teams, focusing on patient-centered care and collaboration to ensure optimal discharge planning and coordination of services.Key ResponsibilitiesCollaborate with multidisciplinary teams to assess and support the needs of the patients and their familiesCoordinate patient discharge planning, ensuring all necessary resources and support systems are in placeConduct biopsychosocial assessments to evaluate patients' needs and appropriate interventionsEngage patients and families in discharge planning processesFacilitate communication and coordination with community partners and resources to support ongoing patient careThis position is not eligible for visa sponsorship with the exception of the TN visa classification; Also, Mayo Clinic DOES NOT participate in the F-1 STEM OPT extension program.QualificationsBachelor's degree in social work (BSW) or master's degree in social work (MSW) is required.Licensure/CertificationLicensed Certified Social Worker (CSW), Licensed Advanced Practice Social Worker (APSW), Licensed Clinical Social Worker (LCSW) by the Wisconsin Department of Safety and Professional Services is required.APSW personnel, following 3,000 hours of clinical social work practice or 4,500 hours dependent upon education credits within the MSW program including at least 1,000 hours or 1,500 hours respectively of face-to-face contact supervised by a person meeting board requirements, must apply for licensure at the LCSW level. Benefits Eligible Yes Schedule Full Time Hours/Pay Period 80 Schedule Details Eight - hour shifts with start times between 7:30 a.m. and 8:00 a.m. depending on unit of coverage. Schedule is 10 days per bi-weekly pay period. Holiday coverage required.
