Nurse Educator

Role Overview: The Diabetes Nurse Educator is a vital member of a multidisciplinary care team, responsible for supporting patients through education, intervention, and clinical coordination in diabetes management. Key Responsibilities: Assess patients for diabetes-related complications, educate on self-management and lifestyle changes, develop group and individual education programs aligned with ADA guidelines, monitor intervention effectiveness, and instruct on medication and device usage including insulin, CGMs, and pumps. Data Review & Device Support: Interpret blood glucose logs, assist with device troubleshooting, and perform CGM sensor downloads for provider review. Care Coordination: Collaborate with families and interdisciplinary teams across care settings, ensuring compliance with health standards and promoting diabetes awareness through community outreach. Professional Development: Stay current with clinical best practices via ongoing education and support quality initiatives in diabetes care. Qualifications Minimum 2 years healthcare experience with proficiency in RN curriculum areas. Bachelor's Degree in Nursing, NY State RN license, and current CPR certification. Certified Diabetes Educator within 2 years of hire. Preferred Skills & Environment: Experience in primary or specialty care, valid driver's license, and additional certifications are advantageous.
