Peritoneal Dialysis Nurse

Provide comprehensive nursing care to patients receiving peritoneal dialysis (CAPD and APD) Educate and train patients and caregivers on home dialysis techniques, infection prevention, and equipment use Perform initial and ongoing patient assessments, including physical, psychosocial, and environmental evaluations Develop, implement, and update individualized care plans in collaboration with the nephrologist and interdisciplinary team Monitor patient progress, labs, and treatment adequacy; communicate changes to the provider Manage and troubleshoot PD-related complications (e.g., peritonitis, catheter issues) Coordinate care with physicians, dietitians, social workers, and other healthcare professionals Ensure compliance with CMS regulations, infection control policies, and organizational standards Maintain accurate and timely documentation in the electronic medical record Participate in quality improvement initiatives and performance monitoring Provide on-call support as required Education Associates or better in Nursing. Associates or better in NURSING- ADN.
