Case Manager, Recuperative Home Homeless Services

US Career Partners

US Career Partners is seeking a qualified Case Manager for a Recuperative Facility in San Diego. The role focuses on individualized support and resource coordination to help clients achieve stability.

You will assess needs, develop care plans, advocate for access to housing, healthcare, and social services, and provide crisis intervention when required. Strong empathy and organizational skills are essential.
