The right candidate will provide expert support, education, and reassurance to patients and their families, while maintaining the highest standards of confidentiality and trust.

Key Responsibilities:

• Conduct comprehensive evaluations and continuous assessments of patient needs.
• Provide direct patient care.
• Exhibit expertise in essential skills for outstanding end-of-life care, including pain management and wound care.
• Develop, implement, and revise care plans as needed to ensure the highest quality of care.
• Collaborate effectively with healthcare professionals to guarantee continuity of care.
• Be a strong advocate for patients and their families, ensuring their voices are honored.
• Educate on Advance Directives and oversee home health aides with assurance.
• Engage actively in interdisciplinary meetings.

Requirements:

• Reliable transportation and a valid driver's license.
• Current RN state licensure.
• Strong scheduling and management capabilities.
• The ability to make sound nursing judgments in diverse field settings.
• Proficiency in assessing patient needs and creating tailored care plans.
