Schedule and Hours:

• Contract assignment; full-time schedule expected

Key Qualifications:

• Active and unrestricted LPN license
• At least one year of recent experience in home health nursing
• Proficiency with HomeCare HomeBase (HCHB) EMR system
• Strong clinical assessment, patient education, and documentation skills
• Reliable transportation to conduct home visits
• Self-motivated with the ability to manage caseloads independently

Primary Responsibilities:

• Deliver skilled nursing care based on individualized plans of care in patients homes
• Monitor, assess, and document patient status and response to interventions
• Administer medications and perform ordered treatments within state guidelines
• Educate patients and families on disease prevention, care strategies, and home safety
• Maintain comprehensive and accurate records within HCHB EMR
• Communicate and coordinate with RNs, therapists, and other healthcare providers to optimize patient outcomes
