Job Summary We are seeking a Care Coordinator to join the transitions of care case management team. In this member‑facing role you will partner with an RN case manager to guide individuals through transitions, ensuring timely, coordinated services following hospital or skilled nursing facility discharge. Schedule & Location Monday - Friday, 8:00AM - 5:00PM Pacific Time (flexible start 7:30AM – 8:30AM). No overtime required. Fully Remote – candidates must be based in California or the Pacific Time Zone. General Duties / Responsibilities Create cases, tasks, and complete documentation in the Case Management module for all hospital and SNF discharges. Reach out to members by phone to assist with referrals, authorizations, home health care, durable medical equipment, medication refills, and scheduling provider appointments and follow‑ups. Request and upload medical records from PCPs, specialists, hospitals, and other providers, including discharge summaries. Work with the RN Case Manager to engage and manage a panel of members. Manage new alerts and update the Case Manager of changes in condition, admission, discharge, or new diagnoses. Complete and document tasks assigned by the nurse. Build relationships with members, earn their trust, and act as patient advocate. Escalate concerns to the nurse if members appear non‑compliant or if a change in condition occurs. Assist with outreach activities to members across all levels of case management programs. Maintain and update member records. Handle mailing or faxing correspondence to members, primary care physicians, and specialists. Meet specific deadlines by prioritizing tasks in accordance with department policies, standards, and business needs. Maintain confidentiality of information among healthcare professionals. Perform other duties as assigned by case management leadership. Job Requirements Experience: Minimum 1year in care coordination, case management, or transitions of care within a health plan, IPA, MSO, or medical office setting. Experience supporting members with scheduling, authorizations, referrals, and coordination of services (e.g., home health, DME, follow‑up care). Preferred experience with hospital or SNF discharges. Education: High School Diploma or GED and/or 4years of relevant experience in lieu of education. Bachelor’s degree preferred. Training: Medical assistant or medical terminology training preferred. Specialized Skills: Bilingual in English and Spanish. Ability to read and interpret documentation and to write routine reports and correspondence. Strong customer‑relations skills and professional communication. Problem‑Solving & Organization: Effective problem solving, time management, and the ability to work in a fast‑paced environment. Knowledge: Managed care plans, Medi‑Cal, and computer proficiency (Microsoft Outlook, Excel, Word). Typing speed 35WPM (10‑key touch). Mathematical & Reasoning Skills: Basic arithmetic, standard measurement units, and standard reasoning for routine instructions. Licensure: None required. Medical assistant or medical terminology certificate preferred. Essential Physical Functions Regularly required to talk or hear; stand, walk, sit, use hands, handto finger, handle objects, reach with hands and arms, and occasionally lift up to 10pounds.
