Responsibilities Key To Your Success Facilitate new patient referral intake process by acquiring, organizing, appropriately summarizing, and tracking medical records from all providers involved in the current and historical care of an individual patient Recognize that accurate and easily referenced medical records greatly impact the quality, cost, and outcome of care for every patient Build a strong working relationship with the Care Navigation Team, which includes the Care Navigator and Hospital Logistics Expert Learn to read, understand, and honor medical records and practice style of community general veterinary practice teams and the Arista specialty & emergency teams Facilitate the ongoing client/patient journey by managing concise electronic medical records across systems Help develop and improve the electronic tools needed to streamline and create more efficiencies in the provision of advanced care Demonstrate and uphold Arista's Core Values and Hospital Mission Demonstrate professionalism and compassion toward patients, clients, and colleagues, especially during stressful situations Maintain confidentiality of patients, clients, colleagues, and business information Comply with all applicable federal, state, local, professional licensure, safety, controlled substance, and hospital policy and requirements Qualifications & Skills What You’ll Bring to the Team 5+ years of experience working in healthcare - veterinary technician, nurse, or relevant clinical experience Experience in both general practice and specialty/emergency veterinary medicine preferred High proficiency with Electronic Medical Record/PIMS systems as well as email, efax, PDF, and document scanning Must be comfortable with medical terminology, medical records, and diagnostic reports Extremely organized, detail-oriented, and self-motivated for an efficient working style Clear and friendly verbal and written communication skills (in-person, telephone, texting, etc.) Ability to work independently while staying connected to a team Commitment to exceptional customer service, high-quality care, and overall excellence Full-Time, Work from Home Position Frequent sitting Ability to meet any position-specific screening requirements, such as a clinical acumen assessment Must be at least 18 years of age at time of hire #J-18808-Ljbffr
