RN Care Manager — Senior Home Care Leader

BoldAge PACE is seeking a Registered Nurse Care Manager in New Jersey to assess participant care needs, provide nursing interventions, and evaluate outcomes on an ongoing basis. You will work with the interdisciplinary team (IDT) to develop plans of care and delegate tasks to clinic, center, and homecare staff.
