Registered Nurse - Case Management

Location: Largo, FL

Assignment Duration: This is a 13-week contract position.

Schedule: Shift: 8:30 AM - 5:00 PM. Expected to work every other weekend and holidays in rotation with hospital employees.

Job Description: We are looking for an experienced Registered Nurse to join our Case Management team. This position will involve managing patient care activities and ensuring effective discharge planning for patients. The role will require collaboration with interdisciplinary teams and strong communication skills.

Responsibilities:

• Manage case management responsibilities with a patient ratio of at least 25:1.
• Facilitate complex discharge planning.
• Utilize EMR systems including Meditech and Midas effectively.
• Work collaboratively with a team of healthcare professionals.

Required Experience / Certifications / Licensure:

• Current RN license.
• Experience with Midas and Meditech required.
• Experience in case management preferred.
