Physical Therapist (PT)

Schedule: 8hrs

Assignment Details:

• Contract Weeks: 13
• Guaranteed Hours: 40

Requirements:

• Current State License as a Physical Therapist
• Proof of current CPR certification
• Current driver's license. Must have reliable transportation and insurance
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities:

• Perform clinical assessments to identify patient needs
• Document and complete patient care plans, including progress notes and other clinical records
• Plan and provide appropriate treatments for various disorders based on medical referrals
• Assist and instruct patients and families in safe transfer techniques and use of equipment
• Educate patients and families on therapeutic procedures
• Collaborate with interdisciplinary teams and oversee other staff in patient care activities
• Apply infection control principles while delivering patient care
