We are seeking an LPN/LVN/Substance Abuse Counsellor to provide compassionate, coordinated care for adults with co-occurring disorders in the home setting. Under the supervision of the nurse manager, this role supports clients through assessment, medication management, withdrawal monitoring, counseling, recovery education, and concierge care while maintaining accurate electronic health record documentation.
Responsibilities
- Conduct patient assessments for adults with co-occurring disorders.
- Administer medications and monitor client response 24/7 for 5-7 days.
- Monitor withdrawal symptoms and communicate changes in condition.
- Contribute to individualized treatment planning.
- Facilitate counseling sessions and provide recovery education.
- Coordinate care across the at-home detox and mental health setting.
- Document assessments, interventions, and progress in the electronic health record.
- Provide family support and concierge care.
- Discharge planning
