Part-Time Physical Therapist Assistant

One Home Health is looking for a part-time Physical Therapist Assistant to join our home health team in Valparaiso, Indiana. What You'll Do

1. Provide physical therapy services and patient education in the home in accordance with the physician's Plan of Care
2. Complete visits and documentation accurately and within established timelines and agency standards
3. Monitor patient progress and promptly report changes in condition or schedule to the supervising therapist or nurse
4. Collaborate with the interdisciplinary care team, including participation in care conferences and referrals as appropriate
5. Follow all safety, infection control, and equipment-use protocols to ensure a safe environment for patients and staff
6. Maintain strict patient confidentiality and protect sensitive information at all times
7. Comply with all agency policies, as well as state, federal, and professional standards
8. Participate in ongoing training, staff meetings, and quality improvement activities
9. Perform additional duties as assigned

What You Bring

• Current license to practice as a Physical Therapist Assistant in the state(s) of employment.
• Graduate of an accredited physical therapist assistant program approved by the American Physical Therapy Association.
• Minimum of six (6) months experience as a Physical Therapist Assistant.
• Demonstrated written, verbal communication skills, and interpersonal skills.
• Licensed driver with automobile insured in accordance with state/Agency requirements. Reliable transportation with good driving record.
