Nurse Practitioner/Physician Assistant

Position OverviewCommunity Care Physicians (CCP) is seeking a Nurse Practitioner or Physician Assistant to join Community Care Internal Medicine Clifton Park, an established adult primary care practice located within the Clifton Park Health Park.This opportunity offers the ability to work as part of a close-knit team consisting of an experienced Internal Medicine physician and two advanced practitioners dedicated to providing personalized, evidence-based care for adult patients. The practice emphasizes preventative medicine, chronic disease management, care coordination, and building long-term patient relationships. Patients are active participants in their care, supported by a team committed to communication, accessibility, and high-quality outcomes.Located in the Clifton Park Health Park, the practice offers convenient access to on-site laboratory services, medical imaging, women's health services, neurology, weight management, and numerous specialty services, allowing providers to deliver highly coordinated care within one integrated location.
