Case Manager Registered Nurse

Medical Recruitment Strategies

Job Summary:
The RN Case Manager coordinates patient care, discharge planning, and resource utilization to ensure quality care and safe transitions from the hospital.
Key Responsibilities:

Assess patient needs and develop care/discharge plans.
Coordinate care with physicians and the interdisciplinary team.
Perform utilization review and ensure appropriate level of care.
Arrange post-discharge services and educate patients/families.
Maintain accurate documentation and ensure regulatory compliance.

Requirements:

Active RN license.
Acute Care Hospital Case Management experience.
Knowledge of discharge planning, utilization review, and care coordination.
BLS certification (preferred/required).
