Summary:

**Required Experience:**- 2 years of previous OT experience- Not an employee of UnityPoint in the last 12 months**Preferred Experience:**- Home health experience- Epic experience**Certifications:**- BLS**Degrees, Licensure, and Licensure Verification:**- State license (Iowa) [required]**Skills and Qualifications:**- Must have own vehicle - Mileage will be reimbursed- Driver's License [required]- Work history/resume [required]- Skills checklist (must have been completed within 1 year of submission date) [required]- References - 1 supervisory reference required at time of submission [required]**Additional Requirements:**- Must have taxable address outside of 50 miles from facility.
