Mileage Reimbursement What You’ll Do as a Wound Care Registered Nurse: Provide skilled nursing care to patients in their homes with a focus on wound assessment and management Perform comprehensive wound assessments and develop individualized plans of care Provide treatment for a variety of acute and chronic wounds Manage complex wounds, including pressure injuries, surgical wounds, diabetic wounds, venous ulcers, and other skin integrity concerns Perform wound measurements, documentation, and ongoing reassessments Educate patients and caregivers on wound care, prevention, nutrition, medications, and disease management Collaborate with physicians, therapists, home health aides, and other members of the interdisciplinary care team Recommend appropriate wound care products and treatment approaches based on the patient's needs Monitor patient progress and make appropriate changes to the plan of care Complete accurate and timely clinical documentation Help patients and families understand their care and feel confident managing their health at home Who you are as a Wound Care Registered Nurse Previous wound care experience strongly preferred Home health experience preferred, but we are willing to consider strong acute care candidates with significant wound care experience Strong assessment and clinical decision-making skills Ability to work independently and manage your daily schedule Comfortable providing care in patients' homes throughout the assigned service area Reliable transportation and the ability to travel between patient visits About Concordia Visiting Nurses: Since 1881, Concordia Lutheran Ministries has been a place with a mission of caring.
