We are seeking a Licensed Practical Nurse (LPN) to provide home healthcare services in the Alexandria, OH area. Responsibilities include Planning, implementing, and evaluating patient care using the nursing process Initiating and revising care plans based on patient needs Administering medications and treatments, monitoring outcomesResponding to patient condition changes and reporting to physicians or supervisors Providing healthcare education, care coordination, and discharge planning Qualifications Include Current LPN license in the practicing state Valid health certificate, PPD or Chest X-ray, and BLS certification Preferred: one year of LPN experience Skills required: clinical nursing skills, patient assessment, medication administration, effective communication, and adaptability.
