Skilled Nursing Case Manager RN - Care Coordination & Discharge

RadNet, Inc. is looking for a Case Manager SNF RN in Temecula, California. In this role, you'll assess and manage patient care plans in skilled nursing facilities, ensuring that the patients’ medical and psychosocial needs are met throughout their stay. The position includes essential responsibilities such as coordinating treatment, managing documentation, and maintaining patient confidentiality in compliance with HIPAA guidelines.
