Patient Care Navigator

Seamlessassist in the United States is seeking a remote Care Navigator for Cardiac care to support a medically complex CHF patient population. This non-clinical telephonic role works under RN supervision to coordinate care and document per CMS standards. The position is full-time, 40 hours per week, remote within US hours, with $21–24/hour compensation and a W-2 contract.
