Make a Difference Every Day As an Occupational Therapist with Acclaim Home Health, you’ll become a trusted partner in each patient’s recovery journey. Your clinical expertise, encouragement, and compassionate care will empower patients to regain independence, improve their quality of life, and confidently navigate everyday living. Your Responsibilities Evaluate each patient’s functional abilities, strengths, and challenges within their home environment to develop a comprehensive understanding of their needs. Create individualized treatment plans that focus on meaningful goals, helping patients regain independence and improve their ability to perform everyday activities. Deliver skilled occupational therapy services through therapeutic exercises, adaptive techniques, and evidence-based interventions that promote safety, mobility, and functional success. Educate patients and family members on activities of daily living, home safety, fall prevention, adaptive strategies, and techniques that support long‑term independence. Recommend adaptive equipment and home modifications that allow patients to remain safe, comfortable, and confident in their own homes. Collaborate with physicians, nurses, physical therapists, speech therapists, caregivers, and family members to ensure coordinated, patient-centered care. Build trusting relationships by providing encouragement, compassion, and emotional support throughout each patient’s rehabilitation journey. Maintain accurate, timely clinical documentation while ensuring compliance with all regulatory and organizational standards. Monitor patient progress, celebrate milestones, and adjust treatment plans as needed to help each individual achieve their highest level of function and independence. What We’re Looking For We’re searching for an Occupational Therapist who combines exceptional clinical skills with genuine compassion and a desire to make a lasting impact. Qualifications Current Massachusetts Occupational Therapist (OT) license Graduate of an accredited Occupational Therapy program Home health experience preferred, but not required Excellent communication and interpersonal skills Strong ability to build meaningful relationships with patients and families Valid driver’s license and reliable transportation Ability to work independently while effectively managing time and priorities Why Join Acclaim Home Health?
