The case manager performs the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client's care.
Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote quality cost effective outcomes. (CMSA, 2009).
Required Qualifications:
Bachelor's Degree
Upon hire: California Registered Nurse License.
2 years Acute hospital experience, including 1 year of supervisory experience.
Strong knowledge base or experience in specialty area assigned.
