Heartline Staffing is recruiting a Primary Care Physician for a full-time outpatient opportunity. This position focuses on senior patients within a value-based care model, with smaller patient panels, dedicated care-team support and an emphasis on continuity of care and long-term patient outcomes. Physicians care for seniors who commonly have multiple complex chronic conditions, including heart disease, diabetes and hypertension. Each physician has a dedicated care team that includes an MA, scheduler and referral specialist, along with leadership support.

COMPENSATION

• Base Salary: $270,000-$315,000
• Bonus Opportunity: Up to 20% of base salary
• Total potential compensation: $350,000+
• Bonus based on patient satisfaction, living the company's values, HEDIS compliance and reducing hospital sick days

PRACTICE MODEL

• Patients commonly have 3-5 complex chronic conditions
• Average 10-12 patients per day
• Maximum patient panel of 650
• Dedicated MA, scheduler and referral specialist
• Full leadership support

SCHEDULE

• Monday-Friday
• 8:00 AM-5:00 PM
• Call schedule based on the number of providers within the market

PATIENT SUPPORT

• Transportation support through Uber/Lyft for patients who do not drive
• Exercise, art and other wellness-focused activities available to senior patients at no cost to the patient

REQUIREMENTS

• MD or DO REQUIRED
• Board Certified or Board Eligible in Family Medicine or Internal Medicine REQUIRED
• Current, unrestricted, active license to practice medicine REQUIRED
• Current DEA registration REQUIRED
• 2+ years of experience in an outpatient environment REQUIRED
• Proficiency with office procedures including suturing, joint injections, skin biopsies and basic wound care REQUIRED
• Fellowship in Geriatrics PREFERRED
