Description:
Primary Care Physician
Old Fourth Ward (O4W) | Atlanta, Georgia
Serving one of Atlanta's most dynamic and diverse communities.
There's a Better Way to Deliver Primary Care
At Ascend Medical, we believe great primary care goes beyond diagnosing illness and prescribing treatment. It means understanding the whole person and partnering with patients to improve their long-term health.
That's why we've built an integrated model of care where physicians work alongside behavioral health providers, nutrition professionals, Advanced Practice Providers (APPs), care coordinators, and other members of an interdisciplinary team. Together, we address not only medical conditions, but also the behavioral, lifestyle, and social factors that influence health.
We're seeking a compassionate, collaborative Primary Care Physician (MD/DO) to join our Old Fourth Ward practice. This is an opportunity to practice evidence-based medicine in an environment that values collaboration, innovation, prevention, and meaningful patient relationships.
If you're looking for a practice where you can focus on delivering exceptional care while working alongside a team that shares your commitment to improving lives, we'd love to meet you.

Our mission is to improve the health and well-being of the communities we serve by delivering compassionate, high-quality, whole-person care through collaboration, innovation, and clinical excellence.
