We foster programs in the areas of basic, translational, mechanistic and population research.Position SummaryThe FIT Program Community Health Worker will provide outreach, counseling, navigational, and case management services to individuals recently released from incarceration with chronic medical, psychiatric and/or substance use conditions who present at or are recruited by the FIT Program.
