Physical Therapist - Travel Contract

Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from a program accredited by APTA or equivalent. Current, active, and unencumbered New York State Physical Therapist license required prior to start. BLS/CPR certification from AHA or ARC required. Minimum 2-3 years of recent physical therapy experience preferred. (Preferred) Experience providing in-home therapy or working in Home Health settings required. Experience supervising and collaborating with PTAs and home health aides preferred. Ability to perform patient evaluations, develop and update plans of care, and document patient progress in accordance with physician orders and state regulations. Reliable transportation, valid driver’s license, and agency-required liability insurance for home visits. Proficiency with basic computer skills including electronic documentation, email, and online training platforms. Assignment Details Facility Type: Home Health. Shift Name: Evenings. Shift Pattern: 5x8. Assignment Type: Travel, temporary help. Start/End Window: Assignment runs from mid-May through mid-August (assignment length provided by client). Clinical Oversight: Provides skilled physical therapy per physician plan of care and may supervise PTAs and home health aides as assigned.
