About the Role

As a Social Service Assistant, you will play a vital role in coordinating and providing psychosocial support to residents and their families. You will assist with admissions and discharges, provide emotional support and crisis intervention, facilitate care planning, and advocate for residents' rights and personal preferences. You will also help identify and address resident needs, including mental health, behavioral, or social issues.

Key Responsibilities

• Assist with admissions and discharges by coordinating assessments, documentation, and family communications.
• Provide emotional support and crisis intervention for residents and families.
• Facilitate care planning and interdisciplinary meetings.
• Advocate for residents' rights and personal preferences.
• Help identify and address resident needs, including mental health, behavioral, or social issues.
• Document assessments, progress notes, and social histories accurately.
• Coordinate referrals to community resources and support services.
• Participate in developing and implementing individualized care plans.

Qualifications

• High school diploma or equivalent required; associate or bachelor's degree in social work, psychology, or human services preferred.
• Prior experience in a long-term care or healthcare setting strongly preferred.
• Knowledge of state and federal regulations for nursing homes a plus.
• Empathy, patience, and excellent communication skills.
• Ability to maintain confidentiality and professional boundaries.
