Ability to manage care for both acute and chronically ill cardiac and pulmonary patients. Ability to develop, implement, and modify patient care plans. Skill in medication administration and evaluating medication effects. Skill in preparing patients for procedures and managing post-operative care. Skill in IV venipuncture and blood administration. Ability to document care accurately using electronic medical records. Ability to educate patients and families on care plans and discharge instructions. Ability to provide constructive feedback and assist in professional development of peers. Ability to handle conflict management and maintain professional communication with team members. Skill in critical thinking to assess patient status and take appropriate action. Ability to collaborate effectively with interdisciplinary team members. Ability to prioritize patient safety and adhere to all safety standards including fall precautions and pressure ulcer prevention. Ability to complete patient bedside shift reports, hourly rounding, and maintain updated patient communication boards. Ability to delegate appropriately and follow up on delegated tasks. Description: The Clinical Resource Unit is committed to a Nursing Professional Practice Model, emphasizing caring relationships with patients, families, and colleagues to provide exceptional care. In this unit, dedicated nursing staff, nurse leaders, interdisciplinary team members, and physician partners provide specialized medical and nursing care across various inpatient units. Medical diagnoses include a range of conditions such as coronary artery disease, angina, myocardial ischemia, myocardial infarction, dysrhythmia, syncope, congestive heart failure, strokes, cancer, cellulitis, post-surgical care, and chest pain. The environment is fast-paced, exciting, and continuously evolving. Responsibilities include performing duties required by the unit specific to the scope of service and implementing procedures related to the specific area of practice. Nurses maintain up-to-date documentation of nursing care provided to assure integration of information for the healthcare team. Initial patient assessments upon admission are completed to develop appropriate plans of care based on patient and family needs. Plans of care, nursing interventions, and patient care procedures are implemented and modified based on continuous evaluation. Nurses take responsibility for patient safety by reporting and correcting safety hazards and incidents. Communication with others is maintained to resolve issues directly and privately when necessary. Constructive feedback is offered to assist in professional development, and colleagues are consulted to expand knowledge bases. Nurses identify their learning needs and proactively seek solutions. Requirements of departmental competencies are fulfilled. Responsibilities also include floating to various inpatient units, providing direct nursing care, and collaborating with multidisciplinary team members for patient care across diverse patient populations. Conducting and documenting assessments, nursing diagnoses, plans, implementations, and evaluations of patient care is essential. Nurses act as resources to staff from various units and coordinate care with multidisciplinary team members to provide quality patient care. Care for acute and chronically ill cardiac and pulmonary patients is managed. Patient goals of care are collaboratively identified, with care plans designed and maintained with patients and families. Nurses implement interventions as needed, including medication administration and evaluation of effects. Patients are prepared for procedures, pain management is provided, and IV venipuncture, blood administration, and laboratory monitoring are performed. Patient discharges and instructions for safe transitions from the hospital are managed. All care is documented using electronic medical records. Team huddles are ensured to occur, and attendance is maintained. Delegation of tasks is executed with appropriate follow-up. Patient call lights are answered promptly to meet medical needs. Basic care, such as turning and toileting, follows established standards. Conflict management is handled while maintaining professional communication with team members. Post-operative care for complex surgical populations is provided.
