Sunnyvale Gardens Post Acute, Sunnyvale, CA, Pay Range: $57$67 per hour

Position Summary

Sunnyvale Gardens Post Acute is seeking a PRN Physical Therapist to join our skilled nursing and post-acute care team. The ideal candidate is compassionate, clinically strong, and committed to helping patients restore function, mobility, and quality of life.

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercise, gait training, balance training, and functional mobility interventions
• Document evaluations, treatments, goals, and progress in accordance with regulatory standards
• Collaborate with an interdisciplinary care team to optimize patient outcomes
• Educate patients and families on treatment plans, safety, and home exercise programs
• Maintain compliance with state, federal, and facility policies

Qualifications

• Current California Physical Therapist license in good standing
• Graduate of an accredited Physical Therapy program
• Experience in skilled nursing or post-acute care preferred
• Strong communication, clinical, and documentation skills
• Ability to work independently and as part of a team

Schedule & Compensation

• PRN / Per Diem (flexible scheduling)
• $57$67 per hour, based on experience
