Requirements:

Must be Board certified in Orthopedic Trauma Surgery or a Truly board eligible - REQUIRED
Must have fellowship in Orthopedic Traumatology or Adult Reconstruction/Total Joint Arthroplasty - REQUIRED
Have an active license in both Iowa and Illinois - REQUIRED
Strong clinical and surgical expertise with a commitment to comprehensive hand and upper extremity care - REQUIRED
Open to both graduating fellows and experienced orthopedic surgeons
Does the provider require visa support - H-1B cap exempt (Note in Additional Submission Details) - REQUIRED
Submit Self-Query NPDB pulled within 30 days from submission at time of presentation- REQUIRED

Two-year guaranteed salary: $739,410, plus production beginning at 10,650 wRVUs
